                                          Why does skin age?

When you are young, your skin is thick and elastic-high in Collagen and Elastin  fibers. As one grows older, the body slows down the production of the essential ingredients it requires to maintain its youthful appearance and elasticity. As a result, the skin becomes thinner and more susceptible to wrinkles, sagging skin, bruising and tearing of the outer layer.

Aging is a fact of life, photo aging id that damage done to the skin due to the direct effects of ultraviolet light. clinically the person will present with skin that has sallow complexion, with surface mottling, and a coarse thickened texture. The skin will often sag or droop due to loss of integrity of the elastic tissue. Wrinkles will be major cosmetic concern, brown spotting (solar lentigines), telangiectasia as well as actinic and seborrheic keratosis.

Solar elastosis ( clinical photoaging) consists of deposition of massive amounts of abnormal elastic material, in the upper reticular dermis. The abnormal elastic fibers being deposited are accompanied by massive amounts of chondroitin sulfate molecules ( versican). This increase in versican and abnormal elastic fibers are accompanied by a corresponding decrease in amounts of collagen and a collagen associated proteoglycan.

What the mechanism of formation of solar elastosis? the primary mutagen seems to be ultraviolet light, exposure to ultraviolet light type A (UVA 320-360 nanometers) led to an increase in free radicals which in turn led to an up regulation and synthesis of interstitial collagenase which would increase collagen degradation and connective tissue damage.

What is available to help combat these forces of ageing? 

1- First and foremost, use of sunscreen.

2- Second, applying topical Tretinoin, which showed to decrease facial wrinkles if used for over 3-4 months. Tretinoin 0.05% cream, marketed as Renova has been FDA approved for this use, however, this is consider a drug and can be only obtained by prescription, a less concentration are available over the counter, such as our Renew Va Rejuvenating Program . In terms of its effects on photoaged skin, Tretinoin has been found to induce a smoothing of the skin by means of epidermal and dermal changes. The epidermis responds within the stratum corneum compaction, thickening of the epidermis and glycosaminoglycan deposition. Effacement of wrinkling has been linked to new collagen synthesis. one study showed that patients using Tretinoin were biopsied prior to treatment and again at 6-12 months. the specimens were studied with electron microscopy and were compared in blinded manner to placebo treated controls. After 12 months the disorganized collagen bundles were replaced by well organized collagen bundles.

3- third, AHA ( Alpha Hydroxy Acid ) is another rejuvenating agent frequently used by cosmetic surgeons and dermatologists. Histologically, alpha hydroxy acid, have been shown to increase the thickness of the epidermis, as well as cause increased collagen density, improve elastic fiber quality, increase papillary dermal thickness and increase dermal acid mucopolysachharides, translating clinically into thicker, healthier skin.
